You can click on "Patient Information Form" and download a PDF form and fill out the form by hand. or You can click on "Fill out your form and Save it or Print it" and fill out the form on your computer or your phone If you are using I phone you need to download application Adobe Reader. Once the form comes up open it in Adobe Reader and enter all the information. This form is NOT stored on any server, but you may  save it or print it. You can also click on "Word - DOC icon and edit form in word.
